If you’re able to print off the below paperwork, fill it out, and bring it with you, that can save us some time at your first treatment but no worries if that is not in the flow.

Location: Please note that I have two locations:

On Wednesdays , Thursdays, and Fridays I work at the Water Tower Place, 5331 SW Macadam Blvd in Suite 129. You can park in the visitor parking lot and come in through the main doors. If you walk straight ahead, you’ll see stairs and an elevator. You will take one of these methods to go down to floor one and turn right. I’m a few doors down on the right hand side.

On Tuesdays I work at Awakening Wellness Center, 1016 Se 12th Ave, Portland, OR 97214. This location has street parking available and one can enter up the front stairs of the house or following the walkway down to the basement door on the left. We will make arrangements before you arrive so you have the door key code.

Paperwork: If you have a printer and can fill out the Health Intake form before arriving to your first appointment, that can save us a little time but no worries if not.

Cancellations: If you need to cancel or reschedule your appointment, please give 24 hour notice.
